WE ARE HERE TO SUPPORT YOU!

Please review the information on this page for information and resources to assist with your PCS move, retirement or if you are leaving military service.  

At the bottom of the page you will find a Questionnaire.  In order to clear ACS you will need to complete the required form and submit.   Once your information has been processed by the ACS staff you will be cleared in the system and will receive a confirmation email from ACS that you have been cleared.   If you do not receive a conformation email call ACS at 757-878-3638.  Please allow 24 hours for your forms to be processed.  

If enrolled in EFMP complete the EFMP PCS Coordination Letter at the bottom of the page and submit, you will be contacted by the EFMP Family Support Staff for a warm handout to your gainning location.

Out Processing Questionnaire

Please complete and submit the Questionnaire and the Exceptional Family Member Program (EFMP) PCS Coorination Letter, if enrolled, then review the helpful information on this page. Once your information has been processed by the ACS staff you will be cleared in the system, after which you will receive a confirmation email from ACS that you have been cleared.

Questionnaire (REQUIRED)

PRIVACY ACT STATEMENT- AUTHORITY: Title 10 USC, Section 3012. PRINCIPLE PURPOSE(S): To collect data necessary to enroll DoD personnel and their family members in the Army Community Service client database. This form is also used as a tool to aid in delivery of services to DOD personnel and their Family members. DISCLOSURE: Failure to provide required information may result in the inability of Army Community Service to provide appropriate profession and /or development services to the individual.

Section 1

If yes, complete DA Form 7415 and the PCS coordination letter.

Soldiers in ETS or retirement status: Complete section 1 only. See Transition section on ACS Out-processing homepage for available information and resource.

National Guard/Reserve Soldiers: Complete section 1 only.

Soldiers in PCS status: Complete section 1 and 2

Soldiers in TDY, Deployment or Remote Tour status: Complete section 1 and 3

MEMORANDUM FOR Gaining Exceptional Family Member Program (EFMP) Manager SUBJECT: Assignment of Sponsor with Exceptional Family Member(s)

1. The EFMP Sponsor listed below has been assigned to your installation.

2. Point of contact is Merlin Choice at (757) 878-3638/DSN: 826-3638 or merlin.choice.2@us.af.mil.

I, the Sponsor, hereby authorize release of my EFM information for the listed family member(s) to the gaining unit mentioned above.
