At the bottom of the page you will find a Questionnaire, the EFMP Form 7415 and the EFMP PCS Coordination Letter. In order to clear ACS you will need to complete the required forms and submit. Once your information has been processed by the ACS staff you will be cleared in the system and will receive a confirmation email from ACS that you have been cleared. If you do not receive a conformation email call ACS at 757-878-3638. Please allow 24 hours for your forms to be processed.

Please complete and submit the Questionnaire and the Exceptional Family Member Program (EFMP) information, if applicable, then review the helpful information on this page. Once your information has been processed by the ACS staff you will be cleared in the system, after which you will receive a confirmation email from ACS that you have been cleared.
