Dependents coming into DEERS to receive an ID Card must have the sponsor with them or the sponsor may pre sign the DD1172-2 and have it notarized or digitally signed. The dependents must bring the DD1172-2 with them unless the form has been saved in the DEERS system when digitally signed.
- Everyone 18 years and older are required to have 2 forms of unexpired ID, bearing the same name. (Expired Driver License cannot be used). Secondary ID can be a state ID, passport, social security card.
- Age 10-17 should have their social security card with them.
